Looking at how blurred the eyes are, I know that I had all the focus points active which picked up that the shoulders were closer and hence focussed on that instead.

Gorgeous eyes. The focus isn't bad, I may not have realised if you hadn't pointed it out. Its the same problem I'm having. On the weekend I was switching between all points and the centre point, still with mixed results. Maybe my hands aren't steady.
I was trying to get the funnel of a spider web over the weekend. Then hand-shake was downright scarey!
do you have a focus beep? it's a bit annoying but sometimes it can help especially with closer shots like this one.

Nope, no focus beep here! Maybe that is something I have not turned on. There are many things in my life that I have been unable to turn on. Add this to the list.
